All original documents for students' Individualized Plans are filed in
our records room. All parents, Guardians, or adult students must present
a photo ID in order to obtain copies of student files. Additionally, the
parent/guardian must be verified through our Student Information
System (or in the case of foster care, written proof of guardianship
AND photo ID must be provided).
Records requests are received Monday through Friday from
8:00-4:30 pm in our Records Room.Once the Records Secretary
receives requests, it may take up to 48 hours to complete the request.
A typical packet of records sent consist of current Individualized
Education Plan, Determination of Eligibility,
Multidisciplinary/Psycho-educational Evaluation,and any
Ancillary Evaluations. Other school districts can request student records.
These requests must be on official school letterhead in order to
process the request.
All parents of students in special education are required to register
their child at the Records Office before enrolling at their school site.
A temporary registration form is given to the parents to submit
to the student's school site as part of the enrollment process. Parents
are encouraged to bring all Individualized Education Plan documents
and evaluation reports when registering their child at the Records Office.
